- Embalmer's affidavit (notarized)
- Letter of non-contagious disease
- Funeral home letter stating: 1) that only the remains are inside the casket and that they were properly encased in a sealer-type metal casket and zinc-lined shipping box; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) cemetery name and location
- Immediate family member must complete, sign, and submit a Laissez-Passer for a corpse
