1. Death certificate (2)
2. Burial/Transit Permit (must bear raised seal of the health department)
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease (must bear raised seal of the health department or medical examiner)
6. Funeral home letter stating: 1) that only the remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) cemetery name and location
7. ALL documents must be notarized, authenticated by the county clerk, and then presented to the State Department for Apostille
8. Casket - sealer-type metal
9. Wood shipping box
