REPATRIATION SHIPPING REQUIREMENTS:

1. Death certificate (bearing health department raised seal; apostille from State Department)
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Funeral home letter stating: 1) only remains are in the casket; 2) all statistical information; 3) consignee's name, address, and telephone number; 4) cemetery name and location
6. Letter of non-contagious disease
7. Letter from physician stating: 1) cause of death; 2) cause of death was not contagious
8. Upon presentation of the above documents, the Belgian Embassy will prepare a Laissez-Passer (from the French word meaning "let pass") which will accompany the remains along with the death certificate which bears the apostille from the State Department.
