REPATRIATION SHIPPING REQUIREMENTS:

1. Death certificate (2)
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease
6. Funeral home letter stating: 1) only the remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; 3) cemetery name and location
7. Photocopies (1 set) of all of the above documents that will be placed inside the casket
8. Casket - sealer-type metal OR wood casket (see below)
9. Zinc-lined wood box is wood casket is used; wood shipping box if metal sealer is used
