REPATRIATION SHIPPING REQUIREMENTS:

1. Death certificate
2. Passport of decedent
3. Burial/Transit Permit
4. Embalmer's Affidavit
5. Letter of non-contagious disease
6. Letter from physician stating the cause of death
7. Funeral home letter stating: 1) That only the embalmed remains are in the casket, 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) Australian funeral home name, address amd telephone number; 5) cemetery name and location
8. Casket - no restrictions but sealer-type metal casket preferred
9. Zinc-lined wood shipping box
10. Consulate will issue a quarantine certificate within 24-48 hours after paperwork has been filed
