4. Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) cemetery name and location
5. Embalmer's affidavit (notarized and authenticated by the County Clerk)
6. Letter of non-contagious disease (notarized and authentized by the County Clerk)
7. Casket - Metal sealer-type
8. Wood shipping box
